Non-adherence to care plans (particularly medication non-adherence) including incorrect usage of inhalers, sudden symptom exacerbations, and 30-day readmissions

what we did

Approach

Utilizing remote patient monitoring devices on an AI-enabled platform, gather signals of risk to detect likely exacerbations before patients themselves can:

  • Connected Care: Real-time monitoring of patient conditions and medication use.
  • Collaborative Management: Empowering patients to actively participate in their care.
  • Smart Escalation: AI-driven system alerts PDNs only when necessary, optimizing their time.
  • Hyper-personalized Nudges: Tailored nudges based on individual motivations, abilities and preferences, to boost engagement and adherence

This recently launched program targets recruiting 2500 COPD and Asthma patients, half of whom are being equipped with sensors to gather risk signals from inhalers, spirometers, pulse oximeters, and other personal devices. The median age of COPD patients (80% of enrollments so far) is currently 69 Y and asthma 51 Y. Adherence rate: 92% - corroborated independently through digital devices

An Ongoing Affair

This ongoing program is the first of its kind – a totally integrated, remote monitoring system for respiratory care of chronically ill COPD patients. Anecdotal data indicate avoided readmissions, improved reliever usage, declining shortness of breath, etc

Patient Satisfaction

Engagement rates in this program have remained much higher than in other remote chronic care programs on the platform despite high median age of participants. Patients learn to manage their conditions better and track key health metrics
