Problem Addressed

Develop a digital health care journey for metabolic health for patients managing Type 2 Diabetes addressing whole health – physical activity, diet management, stress management and medication management.

what we did

Approach

This program integrates clinical and behavioral risk signals to identify patient phenotypes. Using MyMoBeMap as an active guide, the platform’s AI continuously monitors incoming clinical risk signals, the user's motivation to take action, and their ability to do so. Through personalized risk management and behavior change strategies, participants receive tailored nudges to manage their diabetes based on what they are both willing and able to do.

  • The program was set to run for 14 months, from Q1 2021 to Q2 2022, and had a diverse group of 180 participants. This included 112 individuals diagnosed with Type 2 Diabetes (T2D) and 68 pre-diabetics (DPP).

Program effectivness

Clinical Outcomes

T2D participants

MyMoBeMap App group saw a 0.76 drop in median A1C levels as compared to a 0.28 drop in the control group.

DPP participants

While HbA1c levels increased slightly, they remained within the pre-diabetic range, demonstrating the ability of personalized nudging to slow down risk progression of this chronic condition.
