Non-adherence to care plans (particularly medication non-adherence) among hypertensives, pre-hypertensives, diabetics and pre-diabetics.

what we did

Approach

Informed the bottom quartile of poorly adherent, high-risk patients that they had been assigned to a connected care program for hyper-personalized nudging that would help them comply with their care plans. They could opt out of the program (by texting STOP in reply) or register in the program for a personalized nudging experience. ~71% of the invitees remained in the program and chose to register.

The results after three complete years of the program were as follows:

  • Engagement Rates- At least 55% of participants reached back week on week for the 3 yrs.
  • Adherence Rates-MPR (Medication Possession Ratio) ▲ (12%) in this bottom quartile
