“ABC”, a large Dallas based health system with 40+ hospitals
Non-adherence to care plans (particularly medication non-adherence) among hypertensives, pre-hypertensives, diabetics and pre-diabetics.
Informed the bottom quartile of poorly adherent, high-risk patients that they had been assigned to a connected care program for hyper-personalized nudging that would help them comply with their care plans. They could opt out of the program (by texting STOP in reply) or register in the program for a personalized nudging experience. ~71% of the invitees remained in the program and chose to register.
The results after three complete years of the program were as follows:
Down seventy four percent (74%)
Moved from Stage II to Stage I category dropping an avg. of 16 points (Systolic BP)
Moved from Stage I to mild category dropping 6 points (Systolic BP)
~ $2000 higher annual healthcare expenditure on patients with HTN compared with non-HTN
“It's all part of the ABC experience. The coordination of doctors, hospital services and outpatient outreach programs are the first time I've ever experienced this kind of linked program. It makes me feel safe despite my serious health problems . . . I have yet to hear of any other organizations that can compare to ABC.”
